The applicant undersigned is a citizen of India and request that the following information be furnished urgently Under Right to Information act , 2005

Please provide copy of documents requested on a cd/dvd

1.a list of medicines and quantity  supplied to / received by  the hospital  in last financial year

1.b list of items other than medicines,  like syringe, tube etc.  and quantity  supplied to / received by  the hospital  in last financial year

2. a. List of budgets available for purchase of medicines

b. copy of requisitions/intents made for medicine and other items for the current financial year

c. copy of Non Availability certificates issued to purchase medicines using the budget made available by the Local Self Government body or other agencies. 

3. copy of delivery slip / inward challan of medicines and other items received by the hospital or list of medicines and other items and quantities received by the hospital and for the current financial year

4. Officer responsible for maintaining  the stock of medicines and other items in hospital

5. current list of stock of medicines and other items  available in hospital

7. list of medicines  and other items expired or damaged during past 6 months

8. List of medicine and other items the patients had to purchase from outside and total quantity of each medicine prescribed to purchase from outside during the past 6 months

9. a. does the hospital maintain any serial number on slips given to patients to purchase from outside?

9.b. does the hospital have a copy or record of slips given to patients to purchase medicine and other items from outside

9.c. does the hospital have a systems to ensure and check that the items on  slips given to patients to purchase medicine and other items from outside are not in stock and is really needed

9.d. Does the hospital has cctv survailiance where medicines and other items are handled

9.e. Does the hospital has security system to ensure that medicines and other items are not smuggled out .
