I..................... I am a resident of the village. On (Date) under the Employment Guarantee Rule....................... I had applied for a job card. Provide the following details in this regard:
1. Provide daily progress reports i.e. daily progress reports taken on my application. To which officers did my application go and which officer stayed for how many days and what action did those officers take on him during this period? Provide full details.
2. Within the days of applying for an employment job card, the job card should be made? Provide certified copies of the rules or citizens' charter or any other orders/guidelines related to it.
3. Please give the names and designations of the officers/employees who were supposed to process my application but did not take any action.
4. What action will be taken against these officers/employees who are not doing their job properly and harassing the public? When will this action be taken?
5. How long will I get my job card now?
6. How many applications have been received so far from my village for making employment job cards? Provide a list of it with the following details:
a. Name and address of the applicant
b. Application Number
c. Date of application
d. Brief description of the action taken on the application (job card made/ job card not made/ under consideration)
d. If the job card is not made, then tell the reason for it.
f. If made, on which date?
I am paying Rs 10 as application fee separately.

If the information sought is not related to your department/office, then take cognizance of Section 6(3) of the Right to Information Act, 2005 and transfer my application to the concerned Public Information Officer within a period of five days. Also, while providing information under the provisions of the Act, the name and address of the first appellate officer must be given.
