Document dtd 5.9.2019 relating to Registration for Certificate of Addl. Qualification in Odisha Council of Medical Registration proves that she has changed her Surname from "MUND" to "SAHU".

To get admission in Medical College in ST Quota she has submitted Forged ST Certificate.

Caste is determined by BIRTH & can't be changed by ADOPTION. She was born in 'SAHU' family, which falls in OBC & not in ST Category.

Merely because her Adoptive Parents belong to ST category, she shouldn't have been issued with a Scheduled Tribe Caste Certificate.

She has not only deprived the Rights of a ST Student but also taken Financial Benefit as ST Medical Student.

Odisha Government must initiate legal action against Dr. XXX by handing over this case to Odisha Crime Branch at the earliest.
